New Member Application Procedure

To apply for membership, complete the Membership PDF form using the following procedure:

1. Type the requested information directly into the form.
2. Print the form and sign and date where indicated. Scan the form as a pdf.
3. Email or hand the pdf application to a sponsoring active Fellow of the Association, along with your CV, to obtain the sponsoring Fellow’s signature.

4. Send the co-signed application and a copy of your CV by mail, fax, or email to:
